Theresa Tam, Canada's chief public health officer, is concluding her term on June 20th. She began her role in June 2017 and became a prominent figure during the COVID-19 pandemic. In a recent interview, Dr. Tam discussed the job's rewards and difficulties, her experience with a recent deepfake, and offered advice to her yet-to-be-named successor.
Reflecting on her tenure, Dr. Tam described it as an absolute privilege and honor. She never anticipated reaching this position at the start of her career, and she dedicated all her energy to the role, viewing it as a purposeful endeavor focused on serving the public.
The COVID-19 pandemic presented the most significant challenge, being a once-in-a-century event with global impact. The extended duration of the pandemic made it particularly demanding. Being at the forefront, with her voice and face central to the response, was also difficult. While not unexpected during outbreaks, the intensity and duration of the spotlight were challenging, yet essential for communicating with the public.
Regarding the recent deepfake, Dr. Tam acknowledged it could be upsetting. She anticipates such technology will become increasingly sophisticated and believes it's something one must expect and prepare for.
When asked about U.S. Health Secretary Robert F. Kennedy Jr., Dr. Tam emphasized the importance of closely monitoring information, as news and social media are widely accessible. She stressed the need to remain science-based and transparently explain processes to the public.
Addressing the potential erosion of trust during COVID-19, Dr. Tam noted that public opinion research reveals fluctuations in trust in institutions and science. She highlighted the risk of mis- and disinformation on social media and its negative health impacts. She emphasized that the majority of Canadians still trust public health professionals, and this trust should be built upon.
Dr. Tam's advice for her successor includes focusing on core values and purpose during challenging times, such as protecting public health, improving population health, and addressing inequities. She also recommended establishing robust systems and support structures, particularly in preparation for future crises, and building upon the lessons learned from the COVID-19 pandemic.
As for her future plans, Dr. Tam is taking time to reflect and consider her options, recognizing the many choices available to her.

The 'spotlight' was self-inflicted. She sought out the media and then complained about the scrutiny.

Her advice for her successor is useless platitudes. We need real reforms, not more empty words.

It's easy to criticize in hindsight. She made tough calls under immense pressure, and did an admirable job.
